Privacy Policy

This policy explains what Peniel Healthcare LLC (“Peniel”, “we”) collects through https://penielhealthcareservices.com, why we collect it, and what we do with it. It applies to the website only. Information we hold about people receiving care is governed separately by our clinical privacy practices, which we provide directly to clients.

The short version

• We collect only what you type into a form on this site.
• We ask you not to send health information through this website, and our forms are designed so you do not need to.
• We use what you send to respond to you. We do not sell it.
• We do not run advertising trackers on this site.

Why our forms do not ask for health details

A public web form is not an appropriate channel for protected health information. Our referral form therefore asks for an individual’s initials only, and every form asks you to leave out diagnoses, medications, records and similar details. Once we have spoken with you we will set up a secure route for anything clinical. If you do send health information anyway, we handle it with the same care we apply to all client information, but we would rather you did not.

What we collect

Information you give us

• Contact form: your name, email, phone, the role you are enquiring in, your county, the service you are interested in, your preferred contact method and your message.
• Referral form: your name, role, organisation, email and phone; and for the individual being referred, initials, county, services requested, funding source, timeframe and any logistical notes you add.
• Careers form: your name, email, phone, the role applied for, city, availability, experience, certifications, whether you have transport, who referred you and any message you add.

Information collected automatically

Our hosting provider records standard server logs, including IP address, user agent and the pages requested. We use the IP address transiently to rate-limit form submissions so the forms cannot be abused. We do not use advertising or cross-site tracking cookies on this website.

How we use it

• To respond to your enquiry, referral or application.
• To confirm service fit, availability and funding.
• To coordinate with a support team where you have asked us to.
• To protect the website from automated abuse.

We do not sell personal information, and we do not use what you send for advertising.

Who we share it with

Form submissions are delivered by email using a transactional email provider acting as our processor. Contact and referral submissions go to care@penielhealthcareservices.com, and job applications go to careers@penielhealthcareservices.com, so applications are never mixed in with care enquiries. We share information beyond that only where you have asked us to (for example, coordinating with your SSA or guardian), or where the law requires it.

How long we keep it

Enquiries and referrals are retained for as long as needed to respond and to meet our record-keeping obligations. Job applications are retained for up to twelve months unless you ask us to remove them sooner.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Email care@penielhealthcareservices.com or call (513) 393-5166. Some records must be kept where healthcare regulations require it, and we will tell you if that applies.

Security

This site is served over HTTPS with HSTS. Form submissions are validated and rate-limited, and delivered over encrypted connections. No system is perfectly secure, which is precisely why we keep health information out of these forms.

Children

This website is not directed at children, and we do not knowingly collect information from children through it. We do provide care to children and adolescents, but that relationship is established directly with a parent or guardian, not through this site.

Changes

We will update this page when our practices change, and we will revise the date at the top when we do.
