DODD Waiver Services

Certified DODD support that keeps people safe, independent and connected to their community.

    Homemaker / Personal Care (HPC)

    Help with bathing, grooming, dressing, meals, light housekeeping, routines, supervision and community participation.

    Transportation support for approved care needs, appointments, errands, shopping, outings and community activities.

    Participant-Directed HPC

    Flexible support that gives individuals and families more choice while following the approved plan.

    Shared Living

    Home-based support in a safe, stable setting with a family member, caregiver or shared living provider.

    Supported Living

Mental Health Services

Psychiatry and counseling you can reach from home, plus behavioral health integrated with primary care.

    Crisis Intervention & Stabilisation

Program 04

Remote Care Services

Monitoring and care coordination between visits, so problems get caught before they become admissions.

    A Medicare Part B program for patients managing two or more chronic conditions, with a minimum of 20 clinical staff minutes per month.
